(Community-Based Day Support Services helps adults with disabilities to take part in their community and improve their personal skills by providing access to regular meaningful social contact and stimulating activities.)

Kindred Community Services, LLC 

will offer the following supportive services to all:

Males & Females 18 years of age and older, who have been medically diagnosed with a primary intellectual disability.  Additionally, individuals with a secondary diagnosis of any of the following:

Anxiety disorders, including panic disorders, obsessive-compulsive disorder, and phobias

Depression, bipolar disorder, and other mood disorders

Personality disorders

Psychotic disorders, including schizophrenia

Individuals who are visually or hearing impaired

Individuals who are ambulatory/non-ambulatory or, walk with a cane, walker and leg braces, or need support with mobility

Individuals who have Autism as a secondary diagnosis.

Our Services Include:

Various forms of recreational wellness, various cultural and social experiences, as well as volunteer opportunities.

We 

(1:2 or 1:3 staff to individual ratio) Skill Building Services provided in the community as a learning environment. Individuals have an opportunity to engage in meaningful activities throughout Hampton Roads while expanding opportunities to promote personal growth and independence. 

(Educational Services are classes, programs, activities or other services designed to provide appropriate education to a student determined to need assistance.)

(Community Coaching Services is a service designed for individuals who need one-to-one support in a variety of community settings in order to build a specific skill or set of skills to address particular barriers that prevent individuals from participating in activities of community engagement. In addition to skill building, this service includes routine and safety supports)

Activities and public events in the community; Community education, activities, and events; Skill building and support in positive behavior, relationship building, and social skills. Routine supports with the individual's self-management, eating, and personal care needs in the community. Monitoring the individual's health and physical condition and providing supports with medication and other medical needs.

We Provide:

Individuals who need 1 on 1 support in order to build a specific skill or set of skills to address a particular barrier preventing them from participating in community engagement.  This service is a bridge to the individual’s ability to participate in a more fully integrated community engagement service throughout Hampton Roads while expanding opportunities to promote personal growth and independence.

(Transportation Services provide non-emergency medical transportation to/from medical appointments and healthcare facilities.)

Our Services Include:

Transportation to medical appointments to include hospital, dialysis, physical therapy, cancer treatment center, etc.

We Provide:

Compassionate, caring, reliable, and responsible drivers

(Adult Care Home provides seniors with the same services available in large, assisted living communities, but have the luxury of receiving those services in a regular home in a residential neighborhood that is equipped, adapted, and staffed to care for in a smaller setting.)

 We involve you, your family, or designee if desired to develop an Individualized Plan of Care.  Our services are tailored to meet your individual needs.  Our professionally trained staff consists of nurses and aides under the supervision of a Licensed Registered Nurse and direct care of a hospice nurse.  Our caregivers practice COVID-19 Safety Precautions.

Our Services Include:
