Who Qualifies for Home Health Care Services?
Anyone who is under the direct care of a physician. Has frequent falls, trouble breathing, chronic illness, chronic pain, recent hospitalization, or have fear of being alone.
In-Home Hospice Care Services
(Hospice Care Services includes Comfort Care (Palliative Care) to relieve symptoms and give social, emotional, and spiritual support. Patients who receive these services are usually under the direct care of a hospice nurse.)
